Connecting. Growing together. As a Patient Care Coordinator, you operate as an Optum ambassador. Your primary responsibilities include data entry and uploading documents into our EMR and SNF EMR, obtaining info from area hospital EMR. You will also provide operational support, including backfill and care coordination activities. You will put emphasis on how to manage shifting priorities and attention to detail. There will be collaboration and communication across the “Team Model“ with the SCC members in the center. You will be part of the primary care team collaborating with all interdisciplinary team members and will support multiple providers. The Patient Care Coordinator is a flexible team member who works to support nurse practitioners (NPs) and physician assistants (PAs) as well as our patients. This role assists in care coordination activities, administrative tasks and supports our patients and providers.

  • Support interdisciplinary care team with overall member coordination at the direction of RN or NP/PA
  • Data entry and member chart management for: new members, post hospital visits, quality, labs and medications
  • Obtain medical records following hospitalizations or physician appointments
  • Scheduling member transportation or appointments
  • High school Diploma / GED or equivalent work experience
  • Must be 18 years of age or older
  • 3+ years of clinical or medical record experience. Strong knowledge of medical terminology and EHR platforms
  • Access to reliable transportation that will enable you to travel to patient locations
  • Ability to travel up to 100% of the time
  • Reside within Escambia OR Santa Rosa County, FL, or surrounding communities
  • Ability to work full-time, Monday - Friday. Employees are required to have flexibility to work our normal business hours of 8:00am - 5:00pm (no nights, weekends, or holidays)
  • Certified nursing assistant or medical assistant certifications
  • Ability to keep all company sensitive documents secure (if applicable)
  • Required to have a dedicated work area established that is separated from other living areas and provides information privacy
  • Must live in a location that can receive a UnitedHealth Group approved high-speed internet connection or leverage an existing high-speed internet service
