Connecting. Growing together. You will be part of the primary care team collaborating with all interdisciplinary team members and will support multiple providers. The Patient Care Coordinator is a flexible team member who works to support nurse practitioners (NPs) and physician assistants (PAs) as well as our patients. This role assists in care coordination activities, administrative tasks and supports our patients and providers. This position is full-time, Monday - Friday. Employees are required to work during our normal business hours of 8:00am - 5:00pm. It may be necessary, given the business need, to work occasional overtime. Our office is located at 1555 Elm St., Manchester, NH. This is field based in Concord, Manchester, or Portsmouth, NH and surrounding communities. We offer 2 months of paid training. The hours of training will be based on your schedule or will be discussed on your first day of employment.

Requirements

  • High School Diploma / GED OR equivalent work experience
  • Must be 18 years of age OR older
  • 3+ years of clinical OR medical record experience, including knowledge of medical terminology and EHR platforms
  • Must reside in Concord, Manchester, or Portsmouth, NH or within the state of New Hampshire
  • Access to reliable transportation that will enable you to travel to patient locations
  • Ability to travel up to 100% of the time
  • Ability to work Monday - Friday, during our normal business hours of 8:00am - 5:00pm, including the flexibility to work occasional overtime, based on the business need

Nice To Haves

  • Certified Nursing Assistant OR Medical Assistant certifications

Responsibilities

  • Support interdisciplinary care team with overall member coordination at the direction of RN or NP/PA
  • Data entry and member chart management for: New members, post hospital visits, quality, labs and medications
  • Obtain medical records following hospitalizations or physician appointments
  • Scheduling member transportation or appointments
