You will be part of the primary care team collaborating with all interdisciplinary team members and will support multiple providers. The Patient Care Coordinator is a flexible team member who works to support nurse practitioners (NPs) and physician assistants (PAs) as well as our patients. This role assists in care coordination activities, administrative tasks and supports our patients and providers. This position is full-time, Monday - Friday. Employees are required to have flexibility to work our normal business hours of 8:00am - 5:00pm, no nights, weekends, or holidays. This Field Based to cover markets including Dallas and Fort Worth counties. We offer 2 months of paid training. The hours of the training will be based on schedule or will be discussed on your first day of employment.
