As a Field Based Community Health Worker (CHW), you will act in a liaison role with Medicaid members and community partners to ensure appropriate care is accessible. As well as providing home and social assessments and member education. CHW’s work in a team-based structure and spend most of their time in the community engaging directly with members. In this role, you will serve as a Community Liaison and will be the primary point of contact to respond to inquiries and request for resources from Community partners, Members/families, Caregivers, and Providers, and will work collaboratively with the Community Outreach team to address concerns related to the delivery of and access to services for Medicaid members enrolled in the UnitedHealthcare (UHC) Community Health Plan of NM. This position is full - time (40 hours / week) Monday - Friday. Employees are required to have flexibility to work any of our 8-hour shift schedules during our normal business hours from 8am to 5pm. It may be necessary, given the business need, to work occasional overtime. This position is a field-based position with a home-based office. You will work from home when not in the field. Requirements

  • High School Diploma/GED (or higher)
  • 1+ years of experience working with New Mexico community resources
  • 1+ years of work experience in the community
  • 1+ years of experience working with knowledge of the culture and values of the community
  • 1+ years of experience working with and familiarity with the resources available in the community
  • Access to a designated workspace inside the home with access to high - speed internet availability
  • Ability to navigate a PC to open applications, send emails and conduct data entry
  • Currently residing in New Mexico and have lived in the same state for 2 years
  • Must reside within 50 miles of one of these cities: Albuquerque, NM
  • Ability to travel locally up to 30%25 of the time
  • Access to reliable transportation and valid US driver's license

Nice To Haves

  • CHR/CHW Accreditation
  • 2+ years of field-based experience
  • Care management experience
  • Experience working in Managed Care
  • Experience with Justice System or Court System
  • Knowledge and / or experience with behavioral health or substance use disorders
  • Knowledge and Training in SDOH (Social Determinants of Health)
  • Knowledge of Medicaid and / or Medicare population
  • Lived in the state of New Mexico for 5 years

Responsibilities

  • Work in coordination with the Community Outreach Representative
  • Work with Community Based Organizations that provide services, resources and education for Medicaid members
  • Participate in local and statewide Health Council meetings, community meetings and school-based community events
  • Communicate requests, opportunities for collaboration and CBO meetings that Health Plan can participate in
  • Engage Health Plan resource staff – clinical staff, behavioral health services, vendors to assist in access to care and programs
  • Maintain the calendar of regular meetings and distribute it to the Health Equity team and other Health Plan employees
  • Participate and assist in UHC sponsored community meetings
  • Participate and assist in member education meetings
  • Create a positive experience and relationship with the members
  • Practice cultural sensitivity and cultural competence in community efforts
  • Learn and listen to member needs and barriers to help promote self-advocating and resources
  • Provide member education and health literacy on community resources and benefits to encourage self sufficiency
  • Support members to engage in work or volunteer activities, if desired, and develop social support through deeper connections with friends, family, and their community
  • Knowledge and continued learning of community cultures and values
  • Conduct Health Risk Assessments (HRA)
