The Field-based Behavioral Health Case Manager role at Optum, part of UnitedHealth Group, focuses on providing social work services to members in their home settings. This position is integral to the interdisciplinary care team, helping patients navigate the healthcare system and access necessary support services. The role emphasizes preventive care, aiming to improve the quality of life for chronically ill and high-risk patients through comprehensive assessments and care coordination. The Case Manager will work primarily in the field, covering a designated territory, while also having the flexibility to work from home when not visiting clients.

  • Completion of Biopsychosocial Assessments
  • Case management/care management/care coordination/health care navigation
  • Discharge and transition planning
  • Client and family engagement in all aspects of social work intervention
  • Advanced healthcare planning
  • Palliative care, including pain management
  • Hospice and End-of-life care
  • Crisis Intervention
  • Identification of elder/vulnerable adult abuse, trauma, neglect and exploitation
  • Facilitation of benefits and resource acquisition to assist clients and families, including an understanding of related policies, eligibility requirements, and financial and legal issues
  • Master's degree in Psychology, Social Work, Counseling, or Marriage and Family Counseling; OR Licensed Ph.D.; OR Registered Nurse
  • Active, unrestricted, mental health license to practice in the State of Missouri (LP, LPC, LMSW, LMFT, etc.), or an RN with 2+ years of experience in behavioral health
  • 2+ years of experience in a related mental health setting
  • 2+ years of experience working with Microsoft Word, Excel, and Outlook
  • Dedicated work area established that provides information privacy
  • Demonstrated ability to keep all company sensitive documents secure
  • Willing or ability to travel up to 75% in territory listed
  • Willing or ability to work Monday-Friday any of our 8-hour shift schedules during our normal business hours of 8:00am - 5:00pm
  • Live in a location that can receive a UnitedHealth Group approved high-speed internet connection or leverage an existing high-speed internet service
  • Valid drivers license and reliable transportation
  • 2+ years of experience in long-term care, home health, hospice, public health or assisted living
  • 1+ years of clinical or case management experience
  • Experience with electronic charting
  • Experience with arranging community resources
  • Field-based work experience going into homes
  • Background in managing populations with complex medical or behavioral needs
