About The Position

The Family Care Coordinator / Case Manager at KVC Kentucky is responsible for working directly with families to manage and coordinate client care. This role focuses on family preservation and reunification services, ensuring the safety, permanency, and well-being of children and families through in-home behavioral healthcare.

Requirements

  • Bachelors in Social Work (or other related field)
  • Valid Driver's License and Car Insurance

Responsibilities

  • Establish initial contact with families within 24 hours of referral to assess safety.
  • Initiate safety planning and connect families with resources to meet immediate needs.
  • Conduct team meetings and maintain communication with the family and providers.
  • Facilitate the implementation of each family's Action Plan.
  • Make frequent and consistent contact with DCBS personnel.
  • Provide direct services and education targeted at reducing risks, improving child well-being, and preserving child placement with the family per program guidelines.
