The FertilityCare Practitioner provides professional care in Natural Family Planning for Franciscan patients and outpatients, while embracing the Catholic teachings and Ethical and Religious Directives. This essential role provides individualized, integrative gynecological care, following established standards of practices of American Academy of FertilityCare Professionals and the Creighton Model. WHAT YOU CAN EXPECT Maintain certification, proficiency, and standards in teaching the CREIGHTON MODEL FertilityCare System of Natural Family Planning, adhering to all standardized methods built into the Model according to St. Paul VI Institute. Become a member of American Academy of FertilityCare Professionals (AAFCP) within one year of employment. Work cooperatively with multi-disciplinary team, including Director and Manager of FertilityCare Center, physicians, coworkers and patients of Franciscan Health. Provide Case Management, identify patient care needs, and build relationships with patients to assess situation, disposition, and reason for visit. Transfer complex patients requiring higher levels of care and coordination with physicians to RNs employed at Franciscan FertilityCare Center. Develop and maintain relationships with local FertilityCare Practitioners and educators of other methods of Natural Family Planning; provide support to RNs and Manager in promoting Natural Family Planning program within Franciscan Health and local community. Report statistics of program, including, but not limited to, NaProTechnology referrals, follow-ups, etc. to Manager; provide timely documentation of care and maintain patient confidentiality in medical records.
