About The Position

This position will cover Bronx and Manhattan regions. Fee for Service Visit Rates: $65 per visit. Orientation Daily Rates: $165 per day. Requirements

• Master's degree (MSW) plus New York State License required.
• Knowledge of the operations of community-based health and social services programs, concrete services and government entitlements required.
• Must possess good oral and written communication skills along with excellent interpersonal skills.
• Must be able to provide own transportation in order to make visits in assigned area(s) within greater NYC.

Nice To Haves

• Home Care experience preferred.

Responsibilities

• Assesses patient psycho-social issues to ensure adequate delivery of home care services in order to meet agency goals and required objectives.
• Obtains psycho-social history and conducts evaluation and assessment of patient including adequacy of patient support system.
• Provides supportive counseling in regards to emotional, psychological and concrete needs including discharge planning.
• Interprets and provides information about available benefits, entitlements and community programs to patients and family.
• Counsels patients and receommends community based resources regarding their psycho-social needs.
