With a comprehensive range of services supported by an integrated health system, patients can recover in the familiar surroundings of their home. Someone with your depth of compassion, clinical insight and strong decision-making capabilities is the ideal person to map out and oversee the process of a patient’s recovery from illness, injury or surgery at home. You will provide a full range of advanced clinical home care services, including patient assessment, medication management and patient/family education. In this role, you will be sponsible for direct patient care, providing physical therapy treatment to home care patients. You will be responsible for coordinating plan of care with Rehab management and submitting appropriate paperwork on timely basis and abiding by all agency, state and federal rules, regulations, policies and procedures.

Requirements

  • Minimum of Bachelor of Science Degree in Physical Therapy from an accredited institute of learning
  • Registered and Licensed as a PT in NYS
  • Must have a minimum of one year experience as a Physical Therapist in a health care setting; Home Care experience highly preferred
  • Must be able to provide own transportation in order to make visits in assigned area(s); vehicle/drivers license may be required

Responsibilities

  • direct patient care, providing physical therapy treatment to home care patients
  • coordinating plan of care with Rehab management
  • submitting appropriate paperwork on timely basis and abiding by all agency, state and federal rules, regulations, policies and procedures
  • Conducts Start of Care (SOC,) initial and routine home visit assessments and evaluations ofpatient’s therapy and healthcare needs and establishes an inclusive care plan
  • Effectively case manages the care and services for assigned patients resulting in positive clinicaloutcomes
  • Accurately and objectively documents comprehensive clinical findings and patient response totreatment
  • Demonstrates expert knowledge unique to the practice of physical therapy
