About The Position

Family Team Conference Facilitator – Family Treatment & Rehabilitation Program Serving Queens The Family Team Conference Facilitator contributes to the work of increasing safety, reducing risk and promoting wellbeing through the coordination and facilitation of Family Team Conferences (FTC). A FTC is a meeting that convenes the family, informal and formal supports and the case planning team to discuss the family’s strengths and concerns and develop a plan to help families meet their goals while insuring safety and promoting overall wellbeing. The FTC Facilitator works closely with the Case Planners and Supervisors to insure all responsibilities are carried out in accordance with the standards of the Family Treatment & Rehabilitation model of practice, ACS and SCO Family of Services. Requirements

  • MSW or MA in related field.
  • Seeking candidates bilingual in Spanish.
  • Ability to facilitate meetings to address difficult topics sensitively and effectively.
  • Professional demeanor.
  • Some evening hours and flexible schedule required.
  • Excellent organizational, written and oral skills.
  • Proficient in Microsoft Office.
  • Commitment to working from a strength based perspective.

Nice To Haves

  • Prior experience in preventive services.
  • Training in FTC facilitation preferred

Responsibilities

  • Coordinate all Family Team Conferences (FTC): 30-45 Day Conferences, Preventive Services Planning Conferences, Elevated Risk Conferences and Service Termination Conferences, including the attendance and participation of all applicable partners.
  • Facilitate all Preventive Service Planning Conferences when ACS is not responsible to facilitate the conference.
  • Act as primary liaison to the ACS FTC Scheduler for ACS led FTCs.
  • Review case information prior to facilitating conferences.
  • Motivate the participation of all conference participants with the goal of having an open dialogue about the current functioning of the family and to brainstorm ideas for improved family functioning, safety and wellbeing of the children.
  • Assist conference participants to develop written Family Team Agreements with specific and measurable outcomes and tasks.
  • Maintain tracking system of FTC deadlines and completion.
  • Prepare monthly tickler reports on FTC due dates for staff.
  • Prepare monthly statistical reports on FTCs held for program director.
  • Complete all documentation and data entry within deadlines.
  • Flexibility to facilitate FTCs in other SCO locations, families’ homes, etc. as needed.
  • Complete the ACS Family Team Conference Facilitator training
  • Learn the Family Treatment & Rehabilitation practice model and support team to adhere to the fidelity of the model.
  • Attend individual and group supervision and all necessary meetings and trainings.
  • Demonstrate strong sensitivity to cultural and socioeconomic diversity.
  • Commit to being an active contributor to the vision and mission of the program.
  • Work to create and support a respectful learning environment.
  • Other duties as assigned.
