About The Position

The Short-Term Assessment & Respite Home (STAR) program is a 6-bed group home for males ages 11 to 18 years of age. The purpose of this program is to provide short-term care, assessment, and a range of clinical and nursing services to children removed from their homes due to abuse, neglect or other high-risk circumstances. Staff provide empathic professional care for youth within a routine of daily clinical and educational activities. The youth will receive assessment, substance abuse screening, educational support, and intensive work to engage the family. Family Support Worker (FSW) provides services to youth and families in their homes and out in the community. The Support Worker develops a supportive relationship with family members through direct contact in the context of home visits, phone contacts and assistance in times of crisis. Implement the SBIRT, coordinate with educational sources and provides case management as needed. They must work a flexible schedule to accommodate the individual family needs, schedules, and to respond to crisis.

Requirements

  • Bachelor’s Degree in Social Work or related mental health field or equivalent.
  • Valid driver’s license, good driving record, and use of a reliable automobile.
  • Demonstrates culturally competent work history and training.
  • Experience working with children and adults.
  • Must be willing to work with families in their homes and diverse communities.

Responsibilities

  • Provides services to families including parent education, case management, as needed, work as part of the treatment teams with the clinician, facilitate various prevention groups and interact with other service providers as appropriate.
  • Demonstrates understanding of and adherence to the STAR model, and adheres to all contract, certification, and licensing requirements.
  • Gathers and completes all QI and DCF required measures.
  • Documents all services in adherence with state statures, regulatory, billing, COA standards and Boys & Girls Village Standards.
  • Facilitates participation of family members in the youth’s treatment starting at intake. In accordance with the youth’s best interests, the program will facilitate participation of family members, including siblings, in the child’s treatment program.
  • Offer therapeutic support, parent guidance to families.
  • Collaborate with Clinician to provide advocacy, case management and concrete services in accordance with treatment plan.
  • Complete the Screening, Brief Intervention and Referral to Treatment (SBIRT) and provide Brief Intervention.
  • Must be willing to conduct client activities during late afternoon and evening times and/or weekend hours.
  • Transportation of youth as required.
  • Participates in all required trainings, network meetings, DCF reviews, etc.
  • Prevent, intervene, and report instances of child abuse or neglect.
  • Keep Program Manager apprised of all situations which involve the safety of any individuals in the program or community, to monitor the impact on the agency as well as its mission of care for children and families.
  • Perform additional duties as assigned to deliver seamless services throughout the agency.
  • Participate in weekly supervision.
  • Collaborates with other service providers to assure continuity of care.
