The Family Support Worker provides weekly educational home-visiting services to participants and their families who are enrolled in the Healthy Families program. The FSW has to quickly engage families in the program and retain their participation in the program. The FSW is responsible for teaching a research based curriculum based on the learning style of each individual family. The FSW supports and promotes the parent child relationships and promotes positive health outcomes for the targets child by tracking development, immunizations, well baby visits, and emergency visits. The FSW will also conduct developmental screens with the family to assess where the target child is developmentally.

Requirements

  • Experience or knowledge in child development.
  • Experience working with parents and children.
  • Must be capable to sit or stand in front of a computer for long-periods of time
  • Able to work in open space floor plan
  • Must be capable to move throughout work day and follow people served throughout community
  • Work alongside co-workers within 3 feet
  • Must be able to move in tight spaces
  • Occasional lifting of > _25+ pounds

Nice To Haves

  • Bilingual English and Spanish speaking is a plus.

Responsibilities

  • Provide weekly educational home visits to participants and their families.
  • Form trusting and effective relationships with consumers.
  • Assist consumers with positive parent child relationships and positive health outcomes for the child, including medical issues, and any other specific issues that need to be addressed.
  • Track child development.
  • Maintain case documentation including progress notes.
  • Work effectively as part of a service team and with community resources on behalf of consumers
  • Attend workshops and training to enhance skills.
  • Perform other duties as assigned.
