About The Position

The Family Support Worker for Independent Living provides support to children/families with a case plan goal of APPLA to assist them in life skills development, accessing community resources to meet their individual needs, and promote a successful transition from foster care. This role involves providing direct services to youth, supporting case plan goals and transition plans through teaching, role modeling, and demonstration. The worker participates in case conferences, client assessments, and in the team decision-making process. They assist youth in establishing a support network with family, kinship, and community that will be available to youth post custody. Additionally, the worker refers youth to community services and assists in arranging appointments as needed, gathers information for initial and ongoing assessments and transition plans, and ensures that youth have their important documents prior to release from custody such as birth certificate, social security card, and state ID. The role may also involve assisting with the completion of aged out medical card applications for youth and partnering with IL support services staff to ensure youth participate in community activities related to the development of IL skills and RYAC meetings. The Family Support Worker provides intervention for children and families in crisis situations, works with the court system, participates in court proceedings, and maintains written documentation in child/family's case records accurately and within established time frames. They support kinship and foster families to maintain placements and help children work toward permanency, participate in team meetings, supervise and transport children and families as needed, report critical incidents to DCF, and implement Saint Francis Ministries policies and procedures.
