About The Position

Utilizing the Family Spirit Curriculum, this position promotes positive parent-child relationships, supports healthy childhood growth and development; enhances family functioning of parents with children newborn to age five. e in one or more of the CTSI service areas.

Requirements

  • High school diploma or GED and 1 year experience working with families in education or human services, preferably providing early childhood education services to children ages prenatal to 5 years, - OR – an equivalent combination of education and/or experience.
  • Certification as a facilitator for the Family Spirit Curriculum is required within one year of hire.
  • Organizational skills and the ability to prioritize multiple and varied tasks.
  • Microsoft word processing, database and spreadsheet applications.
  • Interpretation of Federal, State and Tribal laws, regulations and policies.
  • Correct punctuation, capitalization, spelling and grammar.
  • Provide strength-based, client centered services in a culturally responsive manner.
  • Work in a team environment and establish positive relationships with tribal members and members of the community at large.
  • Plan and schedule work activities and perform duties with minimal supervision.
  • Communicate effectively both orally and in writing.
  • Work effectively and cooperatively with staff, public, other departments and entities. Establish and maintain professional boundaries.
  • Maintain filing, record keeping and program/project tracking systems.
  • Operate modern office equipment.
  • Read, understand, apply, and explain Tribal/department policies, procedures and 477 Regulations.
  • Work under pressure with the flexibility to multi-task.
  • Travel throughout eleven-county tribal service area and stay overnight occasionally in outlying communities.
  • Must be able to work independently and use ethical judgment in making decisions.
  • Must possess a valid Oregon State driver's license, insurance and be eligible to drive a GSA vehicle.

Responsibilities

  • Deliver parent and child education services in home visit settings to parents and their babies 0-5 years of age according to families’ individual goal plans, families’ needs, developmental stage of baby, and using the Family Spirit curriculum.
  • Determine eligibility for families applying for Home Visiting services.
  • Complete assessments and develop a family case plan. Identify barriers to family self-sufficiency and provide appropriate referrals as needed.
  • Work with families, supervisor, and the Family Spirit program to promote positive parent-child relationships, support healthy growth and development, enhance problem-solving skills and improve support systems.
  • Effectively work with families and individuals of diverse backgrounds (including parents; families with mental health, substance abuse, or domestic violence concerns; families experiencing poverty).
  • Maintain Eaglesun Database for effective case management and to maintain accurate client records, issue notices, case note, issue payments, and scan documents/files.
  • Connect families with community resources and support self-advocacy across systems. Educate community partners and potential clients on Home Visiting services.
  • Assist in planning and implementing an annual client conference. May participate in the development of community and youth prevention events. Develop and implement youth prevention through case management supported under the Federal TANF Purpose (3 and 4) and other activities that support the prevention of generational poverty. Serve as a cohesive member of the team, participate in problem solving and promote innovation.
  • Provide limited support services as outlined in the Policies and Procedures.
  • Maintain accurate, legible, timely documentation detailing contact and progress of families as well as recruitment and retention of families.
  • Complete all required documentation and reports required by program in a timely manner.
  • Complete on-going evaluation, documentation, assessments and training for each family in Family Spirit standards and requirements.
  • Fulfill all training and evaluation requirements as established by Family Spirit standards.
  • Back-up for youth service activities funded by 477-SSP. May participate in Tribal events focused on youth prevention and education, i.e. Culture Camp, Youth Conference.
  • Perform other duties as assigned.
  • Provide lactation education and on-going support to expectant mothers referred by the Siletz Community Health Clinic.
  • Provide and track relevant data for third party billing purposes (if applicable).
  • Provide Full-Spectrum Doula services to expectant mothers referred by the Siletz Community Health Clinic.
  • Provide continuous source of comfort, encouragement and support (both emotional and physical) during labor.
  • Assist with relaxation techniques, breathing exercises, offer advice on labor positions, and act as a mediator and an advocate with hospital personnel. Translate medical terms and explain procedures as needed.
