The Center for Children & Families is seeking a Family Support Specialist to coordinate services and community resources for youth and their families being served in the Family Support and Youth Transition Program, on probation with the Office of Juvenile Justice. You will conduct in-person and telephone interviews with the youth, family, and collaterals. This will lead to weekly meetings, in the community, to gain a picture for a treatment plan to support the family and help the youth complete goals and objectives set forth to meet the requirements of their probation. You will provide learning materials or make referrals to needed family and youth resources. You will foster relationships with outside agencies and collaborate to make sure the youth is meeting expectations on track for discharge from probation. The ideal candidate will need to be a self-starter, proficient in documentation and date base entry, as well as Microsoft Office applications.
