About The Position

As the Family Support Specialist you will provide the supportive services enabling children to safely remain with their family, be reunified with their family, and/or achieve alternative permanency. These activities include provision of family support, care coordination, family functioning interventions, supervision and safety monitoring. Specific responsibilities include: Maintain regular contact with referring worker, family, and identified collaterals to achieve permanency goals. Coordinate interdisciplinary treatment, maintaining weekly contact with the Therapist and Program Coordinator on all assigned cases. Document service delivery, including objectives of treatment and/or intervention strategies. Understand and utilize defined assessment tools in working with all cases. Facilitate community resource planning, case progress evaluation & permanency planning. Coordinate interdisciplinary crisis planning. Participate in discharge planning & documentation. Schedule and facilitate quarterly staffing on all assigned cases. Assess treatment needs. Participate in the preparation and implementation of a service plan with input from client, family, therapist, program coordinator, and referral source. Demonstrate proficiency in use of various counseling techniques. Develop and implement a service agenda for each session. Complete on-call duties as assigned. Calmly and effectively provide crisis intervention in emergency situations. Maintain caseload and consistently achieves quota.

Requirements

  • To qualify you must meet ONE of the following conditions:
  • Posses a Bachelor's Degree from a 4 year college or University, preferably in a social services related field of study.
  • Possess an Associate's Degree in a Human Services related field plus 2 years of related experience.
