About The Position

Requirements

  • Bachelor's degree in Psychology, Social Work, Marriage and Family Therapy, Counseling, or a related field.
  • Previous experience working with clients with mental health and/or co-occurring substance use disorders.
  • Strong verbal and written communication skills.
  • Effective time management skills.
  • Valid Driver's License.

  • Bilingual in English and another language preferred.

Responsibilities

  • Develop case management plans and provide direct case management services to assigned clients and families.
  • Provide front line crisis support to individuals and families via phone, in-home, and community settings.
  • Interpret and adapt guidelines based on situational assessments and consult with supervisors as needed.
  • Provide group, individual, and family counseling with appropriate supervision.
  • Maintain accurate records of counseling and case management activities in compliance with regulatory requirements.
  • Demonstrate knowledge of behavioral health concerns and intervention modalities relevant to the target population.
  • Participate in the development and facilitation of treatment plans and transition plans.
  • Make clinically appropriate referrals for identified support and service needs.
  • Meet direct service productivity goals as defined by the program.
  • Communicate effectively with clients, families, and service providers.
