About The Position

Family Support Partners are responsible for providing peer support to parents and caregivers of children with special emotional, behavioral, and/or mental health needs. This position reports to the Family Support Partner Coordinator.

Requirements

  • High school Diploma
  • Must be a parent or caregiver with a child who has intellectual, mental health, or developmental disabilities.
  • Must have current or previous experience or navigation with PerformCare and Tri-County CMO.
  • A valid driver’s license, reliable vehicle, and current certificate of car insurance, as one will be traveling to families between the three counties: Hunterdon, Somerset, and Warren Counties.

Responsibilities

  • Build supportive relationships with parents and guardians of children receiving CMO services and referred to the Family Support Organization of Hunterdon, Somerset & Warren Counties.
  • Learn, understand, and practice the Wraparound Model.
  • Educate families on values and principles of the System of Care.
  • Support families as appropriate at Child/Family Team (CFT) meetings, by ensuring the family voice is heard and incorporated into the ISP planning and implementation.
  • Provide direct advocacy for families by participating in Individualized Education Plan (IEP) meetings, court hearings, and other meetings as requested by the families.
  • Coach families on techniques on self-advocacy.
  • Assist families in understanding and navigating the child welfare, mental health, juvenile justice, and education systems.
  • Act as a referral resource to parents and caregivers.
  • Other responsibilities as they relate to providing peer-to-peer support.
  • Assist with support groups and events as needed.
  • Maintain detailed written records as required.
  • Other duties as assigned.
