About The Position

The primary responsibility of the Family Support Partner (FSP) is to assist the family/caregiver in accessing services needed to benefit the child and family. The FSP assists the family in navigating the service system to connect to needed services, and assist in educating the family about mental health and community resources. The FSP supports the family as needed throughout the intervention. Work is under the general review of an administrative superior, leaving considerable latitude for independent judgment and discretion within an established framework of policies and state regulations.

Requirements

  • Possesses a high school diploma or equivalent.
  • Experience as a parent, foster parent, or guardian of a child with or at risk of serious emotional disturbance or complex behavioral/special needs.
  • Possesses a valid driver’s license and is, and remains insurable, under Family Guidance Center automobile liability insurance.
  • Knowledge of community resources used in the provision of services.
  • Knowledge and ability to work with persons from diverse cultures.
  • Working knowledge of children’s services.
  • Skill in collaborating with others to produce positive outcomes for clientele and program.
  • Ability to work flexible schedule to accommodate needs of assigned families.
  • Ability to communicate effectively, orally and through legible and clear writing.
  • Ability to assist, and exercise tact, patience, and understanding of others.
  • Ability to work harmoniously with employees, consumers, consumer families, guardians, public agencies, and the general public.
  • Ability to plan and organize working time effectively.
  • Ability to document and complete required paperwork accurately and in a timely manner.
  • Ability to handle confidential information appropriately.
  • Ability to perform with autonomy or with minimum direction.

Nice To Haves

  • Bachelors degree in social work or psychology preferred.

Responsibilities

  • Contacts family within one (1) working day of receipt of the screening form.
  • Assesses the family’s strengths and needs and serves as a liaison to connect to needed services.
  • Coordinates and schedules the completion of the CAFAS assessment.
  • Coordinates and participates in Family Support Team meetings.
  • Links child and family to community supports and services.
  • Acts as a supportive resource for the family.
  • Develops and maintains professional rapport and relationships with consumers; develops and maintains professional, constructive, and cooperative working relationships within the agency and with state and local public assistance and social service organizations.
  • Monitors, evaluates, and records consumer progress with respect to treatment goals; maintains timely, legible, clear, concise, and well organized records of services provided, by using collaborative documentation.
  • Must be able to use the agency electronic medical record program for all documentation. Is required to use collaborative documentation.
  • Provides/arranges transportation for consumers as deemed clinically and programmatically necessary to attend the program and receive needed services.
  • Must adhere to the agency service hour requirements.
  • Remains alert during working hours and complies with attendance policies.
  • Maintains the confidentiality of consumer information.
  • Performs other related work as assigned.
