About The Position

The Family Support Counselor provides family-based mental health counseling, support, crisis-intervention, and case management services to children and families who are referred through social services, probation or mental health departments. Identified clients are in the process of stepping down from an institutional setting into family or family-like setting or at risk of transitioning into an institutional setting from a family or family-like setting. Services will be provided almost exclusively in the community (i.e. family's homes, community centers, schools, etc.). The primary role is to provide direct support to the child, family and team members weekly as needed with the goal of stability and sustainability for the child and family leading toward the goal of permanency.

Requirements

  • Two years of relevant experience working with children, youth and families.
  • Vehicle, California driver's license, and insurance.
  • Fingerprint clearance.
  • Flexible schedule with ability to work early mornings and evenings as needed.
  • Strong organization skills as demonstrated by the ability to effectively manage multiple tasks and attend required meetings.
  • Ability to work within a multidisciplinary team.
  • Ability to work with a diverse population.
  • Cultural humility.
  • Build authentic and trusting rapport with youth and families.
  • Ability to respond to crisis situations.

Nice To Haves

  • Bilingual skills are desired.

Responsibilities

  • Provide one-on-one support to client via intensive home-based services and on-call coverage to the clients and their families as needed.
  • Provider intensive care coordination and case management services consistent with the Wraparound Model.
  • Participation in monthly Child and Family Team (CFT) meetings for each client.
  • Cultivate and maintain relationships with internal and external providers and natural supports.
  • Create Wrap Action Plan documents, taken from notes created at CFT meetings.
  • Collaborate with the entire treatment team, ensuring all members have copies of the Wrap Action Plan and follow up with team members to ensure action steps have been accomplished before next CFT meeting.
  • Attend and participate in weekly supervision, group supervision, and team meetings.
  • Attend all required trainings.
  • Maintain accurate documentation and paperwork including but not limited to mental health progress notes, time sheets, reimbursements, and mileage.
  • Maintain professional behavior at all times, appropriately representing Edgewood's mission and philosophy.
  • Perform all other duties as necessary for the good of the agency as directed by supervisor.
