About The Position

The Family Stabilization Specialist provides crisis mental-health case management to referred cases in order to help stabilize the primary client and, when appropriate, their children and other household members. Services address the full spectrum of high-risk concerns, including domestic and intimate-partner violence, sexual assault, stalking, abuse or neglect of children, elders, and adults with intellectual or developmental disabilities, as well as substance misuse, suicidal ideation, complex family dynamics, co-occurring medical conditions, and severe mental-health disorders.

Requirements

  • Experience providing case management to individuals in crisis (preferably in Colorado).
  • The ability to adapt to a fast-paced working environment that involves constant change and to be flexible and resilient.
  • Broad knowledge of practices, principles, and methods employed in the resettlement of refugees, including an understanding of local mainstream community resources for individuals in crisis and how to access resources.
  • Cross-cultural sensitivity and knowledge with the ability and desire to work with people of other cultures.
  • Outstanding organizational ability.
  • Skills in problem solving, negotiations, decision making and mediating conflict.
  • Computer skills in Word, Excel, and the use of email and other applications.
  • Written communication skills, ability to edit and the ability to make presentations.
  • Ability to explain facts and information to clients for who English is not a primary language.
  • Ability to self-start and work well in a small cross-functional team setting; proven team-builder.
  • Maintains punctuality for work, appointments and report dates.
  • Detail-oriented, extensive documentation skills, organized and able to work in a multi-tasked environment.
  • Possesses a valid Colorado driver's license; has a reliable and consistently available vehicle; and is able to travel throughout the agency's service area by way of interstate highways and primary and secondary roads.
  • Must carry automobile liability insurance at the level of 100,000/300,000/100,000.
  • Demonstrates commitment to the Mission, Values and Vision of Lutheran Family Services Rocky Mountains.
  • Master's degree in social work or other mental health related field with at least one year of experience providing direct case management to highly vulnerable populations in crisis.
  • Applicants with a bachelor's degree in social work or other related mental health field who have at least three years of experience providing direct case management to highly vulnerable populations in crisis will be considered.

Responsibilities

  • Serve as a professional advocate and resource for refugee clients and provide equal treatment to all client groups in accordance with the agency's commitment to the dignity of all persons.
  • Develop individualized Plans of Care to achieve relevant mental health or wellness goals for all FSS Participants through an evidence based, collaborative approach that includes refugee resettlement staff and clients.
  • Assist in identifying and screening referred client needs in order for appropriate services to be arranged.
  • Provide resources and assist with mainstream community services referrals to support with stabilization including mental health, housing, food, public benefits, disability, or employment needs.
  • Provide clients with informal, values-based psychoeducational support and crisis navigation as appropriate, including continued assessment and update of the client's Plan of Care.
  • Provide preventative family-centered, evidence-informed interventions that combine mental-health psychoeducation, mindfulness techniques, and trauma-informed strategies to create Plans of Care that promote best practices, respect cultural practices, and engage the client's own values.
  • Facilitate and provide access to translation and interpretation services as appropriate.
  • Empower and improve client confidence in accessing community resources through skill-building, coaching, and collaborative problem-solving.
  • Actively foster community partnership opportunities to increase client access to mainstream community services, including mental health services, social connections, and stability resources.
  • Work closely with agency staff to ensure crisis case management services are provided through a team approach in a sustainable manner.
  • Maintain professional client case noting of all contacts with the client and document all client services in a timely and accurate manner.
  • Work collaboratively with the FSS team to develop and facilitate specialized staff and community trainings that promote clarity and awareness of refugee mental health and wellness topics.
  • Provide data required for various reports; submit all required reports in a timely and accurate fashion.
  • Abide by high quality of standards of service delivery expectations of the refugee resettlement agency, State agencies, Office of Refugee Resettlement and U.S. Department of State.
  • Ensure that program services are provided in accordance with quality assurance standards of the agency, state, accrediting organizations and HIPAA.
  • Comply with all federal, state and agency contracts.
  • Provide positive team building leadership through communication, cooperation, information sharing and problem solving.
  • Demonstrate an organized and pro-active approach to clients, program reporting and communication to all agency staff.
  • Participate in community outreach activities that advance the work and message of the Refugee and Asylee program.
  • Performs other duties as assigned.
