About The Position

Brothers Property Management (BPM) is seeking a Family Services Coordinator (FSC) who will play a key role at three of our properties located in Aurora within the BPM portfolio and the surrounding greater community. The Family Services Coordinator will provide clients with both educational information and supportive access to local services and resources that can assist them in achieving their housing and service needs. Reporting to the Resident Services Manager, the Family Service Coordinator empowers clients to grow within their home environment and to be as self-sufficient as possible through a mutually collaborative relationship with the program supervisor, community personnel, City of Aurora, and vendors.

Requirements

  • Associate degree-required, bachelor’s degree-preferred, with concentration in social work, family services, human services, or another service-related field.
  • Must have experience working with families.
  • Familiar working with veterans and elderly/disabled populations.
  • Ability to understand and respect values, attitudes, and beliefs that differ across cultures and to respond appropriately to these differences with participants and in planning, implementing, and evaluating programs and services.
  • Build relationships with participants to support their needs and goals.
  • Demonstrates ability to establish trusting, confidential relationships.
  • Strong self-management, multitasking, and organizational skills.
  • Effective communication skills, problem solving and decision-making ability.
  • Must hold a valid state of Colorado driver’s license, insurance, and reliable transportation.
  • Must be able to follow directions, provide excellent customer service, always display respect and professionalism.
  • Must be able to work independently or as part of a team when required.
  • Must be able to manage general office equipment (fax, computer, copier) and mobile phone.
  • Must be able to lift to twenty pounds, walk up and down stairs, stand, sit, lay for extended periods of time.

Responsibilities

  • Responsible for developing the Community Participant service plan which includes intake, assessment, plan of referrals and linking specific service assistance or network of services.
  • Conduct an initial orientation and ensure that each new participant receives an assessment to determine their interest in and need for services.
  • Provide initial intake and ongoing assessments, if applicable.
  • Provide an explanation of the service programs offered and acts as a resource to connecting participants interested in accessing these services.
  • Identify and assess individual needs and include family when appropriate. Inform participants of available resources and provide support in assessing service assistance.
  • Assisting participants with housing resources as needed.
  • Identify, assess, select, develop, and maintain referral partnerships with local service resource agencies that provide appropriate support and assistance to community participants.
  • Develop and maintain a comprehensive resource directory which is utilized by staff.
  • Present the participant examples of service assistance that the Family Service Coordinator can provide community linkages such as after school programs, ESL programming, Veteran assistance, proactive financial assistance, health/wellness education, emotional support, homemaker assistance, and legal issue education/awareness.
  • Collect data to document services provided and outcomes achieved.
  • Participate and coordinate community educational events related to health care, agency support, life skills, referral sources, and others.
  • Promote participant participation in local school programs, community outreach, senior centers, and other organizations.
  • Creating individual electronic files for all participants and assuring all program forms are used and scanned into the system.
  • Completing and/or updating file progress notes documentation utilizing Family Metrics software online database within the 3-day guideline for file completion.
  • Completed all reports in a timely and thorough manner with proper back-up documentation maintained.
  • Development of annual outcome measures that include programs, training, and services that target the distinct needs of various community participant populations. Consistently track and measure program target progress. Work with RSM to analyze and utilize data as the basis for continuous improvement.
  • Work with RSM and the Grant Writing department to pursue avenues for additional funding as applicable through private, local, state, and federal resources for program and participant needs.
  • Travel is necessary to ensure that participant services are fulfilled and that all training requirements are met. Travel may include going to various locations to meet with program participants or vendors.
  • Perform any other additional duties as assigned.
