About The Position
Nexus-FACTS is excited to add a Family Responder for its innovative Family Response and Stabilization Services (FRSS) program! This program crisis provides response and stabilization services to Hennepin County families with youth ages 0-18 everyday, 365 days a year, 24/7! Crisis need is defined by the family and services are driven by family's cultural, community and clinical needs. Our Family Response Team de-escalates and addresses the immediate concern with the goal of responding in-home within one hour. Our Family Response Team connects family with other supports with option of eight-week stabilization services to help promote well-being of the family.
