About The Position

As the first point of contact for families referred to the program, this position completes the initial screening to determine eligibility for services and arranges for services and supports. Provides referrals, tracks resources in the community, and engages community members and/or organizations.

Requirements

  • Effectively communicate via written, verbal, in person and virtual methods.
  • Skilled at maintaining a culture of respect.
  • Able to work both independently and a part of a team.
  • Understanding of social and emotional development for children ages birth to five.
  • Knowledge of appropriate development milestones for children ages birth to five.
  • Bachelor’s Degree in psychology, social work, sociology, education, counseling or a related field required.

  • Infant Mental Health Endorsement Preferred.

Responsibilities

  • Provides ongoing and continuous outreach to build relationships with community referral sources.
  • Creatively engages new community partners and referral sources.
  • Serves as the main point of contact for incoming referrals for the designated area.
  • Maintains required documentation.
  • Will be point of contact for Mother to Mother, orchestrating orders and pickups as well as completing requirements of the partnership such as volunteering.
  • Assist in preparation of required monthly, quarterly and annual statistical reports.
  • Participates in training according to requirements of HFA guidelines, GKI and DOH contract.
  • Conduct chart reviews for team twice a year.
  • Builds and Maintains relationships with community partners through intentional outreach efforts and attendance at community meetings.
  • Maintains and updates resource log on shared drive.
  • Attends training and other mandatory meetings
  • Coordinate parent groups and/or graduations for participants in the community, as well as other support groups (smoking cessation, breastfeeding, etc) as needed.
  • Collects data required by funding source to document compliance.
