About The Position

Family Resource Specialist (FRS) at CHC/SEK are motivated, and passionate about improving health outcomes for pregnant patients of all ages and children/families. This position provides services for anyone experiencing pregnancy, children, or families need of care management. This position completes initial interviews to determine patient and family needs, both health and social. FRS go above and beyond, often assisting patients and their families through innovative methods in nontraditional locations such as a patient’s home or in the community. FRS must be flexible, knowledgeable about women’s health and family support, community resources and CHC/SEK care systems, and able to address barriers to improved health outcomes. This position is part of the Patient Engagement team, which focuses on clinical and non-clinical care management and coordination, benefits enrollment, social service navigation, community outreach, home visits, and administering patient-centered education and programming. The team utilizes evidence-based practices to outreach to defined patient populations and receives referrals from both the clinical staff and community partners to advance key community health indicators.

Requirements

  • Bachelor's Degree in nursing, social service, public health; RN, LPN or related field, required.
  • Completes a Car Seat Certification Technician training course within six months of hire date.
  • Two years' experience in the area of social work or case management to the target population (women, children and families), required.
  • Must be BLS certified within six months of hire date.
  • Able and willing to work on an as-needed basis in order to meet operational needs of the clinic.
  • Valid Driver’s License as travel is required. Must maintain driving standards. No more than 2 minor or 1 major driving violations within a 3-year lookback period.
  • Social and cultural sensitivity appropriate to ethnically and economically diverse patient-and employee-base
  • Demonstrate knowledge of the rationale of appropriate patient care.
  • Communicates through appropriate channels. Use proper chain of command for patient complaints.
  • Ability to handle emergency situations calmly and effectively.
  • Must be computer literate.
  • Must be able to maintain good inter-personal relationship with co-workers and other members of the health care team and the organization.
  • Provide customer service in accordance to the organization’s mission.
  • Be courteous and respectful when interacting with patients and family members.
  • Maintain patient confidentiality in accordance to organization’s policy and procedure and HIPAA requirements.

Responsibilities

  • Provides considerate, respectful care focused upon patient and/or family individual needs.
  • Accepts referrals of expectant mothers of all ages, children/families in need of care management (such as children with special health care needs, etc.), and school-based health centers for children in need of support or follow up.
  • Provides initial intake for prenatal patient (completing OB history, medical history, etc) and following care throughout pregnancy and after delivery.
  • Facilitates appointments for well child, prenatal, immunizations, medical, dental, behavioral health, family planning, STI testing/treatment, etc. and engages patients in the development of health and social goals while coaching patients in the effective management of their health conditions and self-care utilizing techniques such as motivational interviewing.
  • Assist patients in understanding care plans that include clear goals, priorities, and realistic actions to achieve their goals. Support progress by making and attending follow-up appointments, scheduling and providing transportation, accessing prescriptions, or other means as needed.
  • Maintains contact with patients/families at least monthly until services are no longer needed.
  • Provides pre-natal and parenting education on best practices, including breastfeeding, safe sleep, proper car seat installation, etc.
  • Coordinates expectant mothers and children’s medical visits with dental and mental health visits; works with the dental care coordinator.
  • Works with Medicaid providers to ensure children (especially adolescents) are accessing health care per the Early and Periodic Screening, Diagnostic and Treatment (EPSDT) schedule.
  • Meet patients, and community members in the home, clinic, or community setting and conduct needs assessments, and screenings, including identification of socio-economic issues that affect their overall health (Ages and Stages Questionnaire, EPDS, PRAMMS, PRAPARE etc).
  • Assess the patients’ unmet health and social needs to connect with community and organizational resources, including clear guidance about why and how to access these resources.
  • Connects patient and/or family with resources within CHCSEK and community.
  • Maintains appropriate level of direct contact with families per program requirements, and remains available for Outreach Events and patient needs as they may arise.
  • Works as a team member with other care managers/care coordinators to avoid duplication and/or gaps in service provision.
  • Ensures patients and their families, have access to a medical home (medical, dental and behavioral services).
  • Works within the patient care team to ensure health insurance coverage and other community benefits.
  • Organizes and coordinates at least one special activity (with collaborating partners) annually focusing on child health or safety such as a bicycle safety fair, car seat check, etc.
  • Tracks referrals made to other non-CHC/SEK providers to ensure continuity of care and to meet reporting requirements.
  • Participates in interagency coalitions throughout Southeast Kansas; promotes Maternal Child Health (MCH) services to participating agencies.
  • Meets all state and federal grant requirements, including reports and data entered into Daisey in an accurate and timely manner.
  • Attends Kansas Governor’s Conference/ Public Health.
  • Maintains certification by conducting Car Seat Checks and participating in Car Seat Check events.
  • Performs other duties as assigned.
