Southwestern Behavioral Healthcare's Child & Family Services Division is currently seeking a full-time community-based Family Preservation Child and Family Skills Coach to add to our team of professionals. WHAT IS THIS POSITION RESPONSIBLE FOR? Provide community-based case management, skills training/development, and other services to children and their families. Case management includes but is not limited to communicating with all parties involved in the individual/family's treatment; identifying treatment needs and facilitating all necessary referrals; coordinating and attending collateral meetings with all parties involved in the treatment process. Conduct family, individual and group counseling to youth and families. Provide emergency counseling and crisis intervention when necessary. Maintain appropriate records of services to reflect patient progress and document treatment plans and goals. Must meet efficiency standard which include chart management, patient contact and daily production standards. Participate in regular staff meetings and ongoing staff training/development activities. OTHER RESPONSIBILITIES: Conduct consultation and education activities to community and professional groups as needed. Maintain contact with other service providers, community agencies, schools and civic organizations to enhance community awareness and public relations.
