About The Position

Mosaic Community Health is hiring a full-time advanced practice provider (APP) or MD/DO to deliver services directly to patients experiencing homelessness in their own environment such as under bridges, in encampments, and on the street. As a member of an interdisciplinary care team, this provider will help to pioneer the field of street medicine in Central Oregon, home to a rapidly growing community of patients experiencing sheltered and unsheltered houselessness. As part of a multi-disciplinary team, the provider will perform diagnostic, therapeutic, preventative and health maintenance services. In this position you will join a collaborative, multidisciplinary and dynamic team who is committed to delivering comprehensive, patient centered care to patients with complex social, medical, and behavioral health needs, in particular to those with experiences of homelessness. This position provides services to individuals experiencing homelessness in encampments, on the streets, at shelters, stabilization or navigation centers and other public locations. Nice To Haves

  • current Advanced Practitioner License (PA, NP), MD/DO
  • 3 years in community health, experience working with the homeless population
  • Three years of relevant work experience preferred
  • Two years of experience working with patients experiencing houselessness preferred

Responsibilities

  • Provide patient centered, trauma-informed care in non-traditional health care settings.
  • Rotates care setting locations and accommodates a non-traditional schedule that is flexible to the changing needs of a rural and urban houseless population.
  • Acts as an informal leader, who may be the only licensed independent provider in the field.
  • Uses current evidence-based guidelines in creating diagnostic and treatment plans; utilizing tools such as "Up to Date" or "Choosing Wisely."
  • Provides care which reflects health care cost consciousness and addresses social determinants of health.
  • Effectively utilizes team-based care model, leveraging expertise of all members of multi-disciplinary team.
  • Proactively addresses preventative health issues.
  • Acts as patient educator, leveraging tools (e.g. EHR, on-line resources, written materials, MY Chart messages, etc.) to effectively communicate information with patients regarding diagnoses, treatment options, disease prevention, safe health behaviors, self-care skills, and health promotion.
  • Maintains accurate electronic health records of patients and completes documentation in timely manner.
  • Reflects critical thinking in documentation
  • Reviews incoming results/reports (e.g. lab, x-ray, EKG), paperwork, refills, etc., and addresses them in a timely manner.
  • Responds to patient telephone calls and MyChart messages in a timely manner.
  • Accurately codes patient charts to ensure correct billing.
  • Utilizes EHR tools (e.g. "fyi", problem-based charting, etc.) to enable other clinical team members to effectively care for patients
