The Family Practice Physician is responsible for the provision of primary care medical services in a Patient Centered Health Home context, with a focus on access to care. This role involves comprehensive health care including health promotion and maintenance, prevention of illness and disability, and application of a holistic healthcare model. The physician will manage chronic diseases and acute illness, coordinate care with specialists, and utilize community resources when appropriate. The position emphasizes excellent customer service and patient education, while also participating in community events and meetings.

  • Direct provision of primary care medical services meeting all established productivity expectations.
  • Provide examination and review appropriate laboratory, referral, and imaging results to determine a diagnosis.
  • Document visits accurately utilizing appropriate templates and record relevant data to attain a diagnosis.
  • Accountable for attaining budgeted visits and meeting clinical care measures.
  • Lock records in the 72-hour window.
  • Provide comprehensive, continuous, and coordinated medical care for acute and chronic illness at all ages.
  • Advocate access to health care with a focus on structured evaluation, early intervention, and health promotion.
  • Provide excellent customer service and care.
  • Participate in daily huddles and incorporate the participation of ancillary health team members.
  • Assist in the assurance of clinical procedures and maintenance of up-to-date clinical protocols.
  • Complete chart and peer review responsibilities in a timely manner.
  • Assist the patient care team with performance improvement efforts, staff education, and FHCN promotion.
  • Participate in the preceptorship of mid-level practitioners and rotation of medical, physician assistant students, and dental residents.
  • Participate in Network Health Fairs and other community events.
  • Meet Network and credentialing expectations for licensure and employment.
  • Perform other duties as assigned.
  • Doctor of Medicine or Doctor of Osteopathic Medicine degree.
  • Licensed in the state of California as a Family Medicine Physician.
  • Extensive formal training in Family Medicine.
  • Ability to create complex documents in Microsoft Word.
  • Ability to use advanced functions of Microsoft Excel.
  • Ability to develop sophisticated presentations in Microsoft PowerPoint.
