About The Position

Family Health Services is a non-profit, Federally Qualified Health Center (FQHC) providing quality, affordable health care to all. We offer primary care, behavioral health, and dental services at our clinics in Sandusky and Norwalk. We have the unique ability to offer medical services to people from all walks of life to afford high quality health care services. We are looking for a Certified Family Nurse Practitioner to join our team to provide exceptional medical services to the patients of Family Health Services. This position is a full-time day shift position working Monday through Friday in an out patient setting at our Medical Center in Downtown Sandusky. This position works in a team-oriented flexible environment with a commitment to work/life balance.

Requirements

  • Must hold a diploma from an accredited School of Advanced Practice Nursing.
  • Licensed to practice as a Nurse Practitioner in the State of Ohio.
  • Current Nurse Practitioner Certification.

Nice To Haves

  • Nursing experience in ambulatory care setting, community health setting is ideal.
  • 3 years' experience in practice as a nurse practitioner.

Responsibilities

  • Completes patient history and physical and document all patient care provided.
  • Formulates and directs plan of care for the patients and refers to other physicians, specialists as needed.
  • Embraces the integrated care model and provides holistic quality care while collaborating with the whole team.
  • Prescribes medication in accordance with Ohio Revised Code and Ohio Administrative Code and Formulary provided by the Advanced Practice Nursing laws.
  • Complies with State Advanced Practice Nursing Guidelines/Law on Practicing Medicine.
  • Responds to medical emergencies and acts as a leader in these situations.
  • Directs patient education for health education, maintenance, emotional and physical needs.
  • Participates and provides care as according to the chronic disease model.
  • Establishes and maintains professional working relationship with other members of the health care team.
  • Serves as a role model for clinical staff.
