About The Position

Family Health Services is a Federally Qualified Health Center (FQHC) that provides quality, affordable primary care, dental, and behavioral health services. Our "high touch, high tech" clinics offer unique patient focused care to anyone in the community, regardless of their ability to pay. We are rapidly growing in Sandusky, Ohio at our two locations as well as in Norwalk, Ohio and surrounding areas. The Family practice and Women's Health/Prenatal Nurse Practitioner's primary role is to provide medical services to the patients of Family Health Services in acute care and thje clinical office setting in a holistic approach. The right Nurse Practitioner for this role is required to see both primary care patients and Women's health patients until there is enough patients needing to be seen in Women's Health/Prenatal at which point the NP would fully transition to a role the focuses only on Women's Health/Prenatal care.

Requirements

  • Must hold a diploma from an accredited School of Advanced Practice Nursing.
  • Licensed to practice as a Nurse Practitioner in the State of Ohio.
  • Current Nurse Practitioner Certification.
  • Nursing experience in ambulatory care setting, community health setting is ideal.
  • Minimum 3 years' experience in practice as a nurse practitioner.

Responsibilities

  • Completes patient history and physical.
  • Documents all patient care provided according to policy and Standard of Care Agreement.
  • Formulates and directs plan of care for the patient as according to Standard of Care Agreement.
  • Refers to other physicians, specialists as needed.
  • Prescribes medication in accordance with Ohio Revised Code and Ohio Administrative Code and Formulary provided by the Advanced Practice Nursing laws.
  • Complies with State Advanced Practice Nursing Guidelines/Law on Practicing Medicine.
  • Responds to medical emergencies.
  • Directs patient education for health education, maintenance, emotional and physical needs.
  • Participates and provides care as according to the chronic disease model.
  • Participates in Medical Staff activities at FHS.
  • Participates in Performance Improvement/Risk Management Program.
  • Utilizes supplies in a cost-effective manner.
  • Demonstrates effective communication skills.
  • Establishes and maintains professional working relationship with other members of the health care team.
  • Participates in planning and problem solving.
  • Participates in continuing education and professional growth.
  • Attends provider's meetings.
  • Maintains knowledge of, and complies with, personnel and medical, policies and procedures.
  • Assists other team members.
  • Assists with seasonal programs as needed.
  • Serves as a role model for clinical staff.
  • Adheres to the standards and policies of the Organizational Privacy/Security and Compliance Programs, including the duty to comply with applicable laws and regulations (HIPAA, OSHA, OIG, guidelines, other State and Federal laws). This also includes reporting to the Board of Directors, Compliance Officer, Privacy Officer, supervisor, suggestion box, any suspected unethical, fraudulent, or unlawful acts or practices.
  • Performs other duties as assigned.
