The Family Peer Advocate will deliver vital services to pregnant and postpartum women consistent with the Preserving Families Through Partnership model design. In this role, you will be part of a compassionate, collaborative team focused on making a meaningful difference in the lives of high-risk mothers affected by substance use. Your skills in outreach, advocacy, and case management will profoundly impact families within the community, promoting positive recovery journeys and fostering resilience. Your responsibilities will include direct outreach, child and family team coordination, home and community visits, and the development of tailored Treatment Plans. You’ll play a crucial role in coordinating and delivering individualized services, offering recovery-focused case management and rehabilitation support to mothers dealing with various challenges including substance use concerns and environmental stressors.
