About The Position

The Family Peer Advocate will be instrumental in delivering compassionate, high-quality services to pregnant and postpartum women as part of the Preserving Families Through Partnership model. Join our dedicated team to make a profound difference in the lives of high-risk mothers affected by substance use. In this role, you will provide community/home based services including: outreach, coordination of family services, and recovery-focused case management, ensuring that every mother receives the support she needs. You will work directly with families in various settings, fostering positive relationships, advocating for their needs, and guiding them toward recovery and stability. Your commitment to empowering families will help shape a stronger community.

Requirements

• Experience working with families in need, experience working in substance use disorder treatment, or experience in case management is required.
• Lived experience (self, family, friends) in active recovery from addiction to drugs and/or alcohol is a big plus.
• Ability to explore all forms of recovery.

Nice To Haves

• Bachelor’s Degree in psychology, social work, sociology, education, nursing, or a related human service field.

Responsibilities

• Represent our organization in a positive manner to clients, staff, community agencies, and community members.
• Advocate for clients and treat all persons served and their families with dignity and respect.
• Manage assigned caseload of clients and their families, depending on severity level.
• Participate in wraparound process by creating and monitoring individualized Treatment Plans, and deliver services as indicated.
• Develop a strong, non-judgmental relationships with clients and their families, modeling appropriate behavior as a role model.
• Administer assessments as assigned and assist with daily living skills teaching (budgeting, organization, nutrition).
• Facilitate recreational, educational, and vocational activities.
• Provide a minimum of 90 hours of direct client/family service per month.
• Document interventions and collect data in a timely manner, ensuring thorough client record maintenance.
• Work on crisis intervention as needed, utilizing resources to resolve issues.
• Educate mothers on addiction, recovery methods, and community support.
• Use personal experience to build rapport and support recovery efforts.
• Communicate effectively with community professionals and participate in required meetings.
• Attend necessary training, conferences, and supervision appointments.
