About The Position

Our Behavioral Health Department (BHD) offers a comprehensive array of high quality, culturally-sensitive mental health services that provide children and families with the services they need for success at home, in school, and in the community. Position Summary: Family support and training is a service provided to the parent/caregiver of a youth (under the age of 21) in any setting where the youth reside, such as the home (including foster homes and therapeutic foster homes), and other community settings. Family support and training is a service that provides a structured, one-on-one, strength-based relationship between a Family Support and Training Partner and a parent caregiver. The purpose of the Family Partner is for resolving or ameliorating the youth’s emotional and Behavioral needs by improving the capacity of the parent/caregiver to parent the youth as to improve the youth’s functioning as identified in the outpatient or In-home therapy treatment plan or Individual care plan (ICP).

Requirements

  • Experience as a caregiver of a youth with special needs and preferable a youth with mental health needs. (Family caregiver refers to any biological, kinship, foster and/or adoptive family/caregiver responsible for the care of a youth).
  • Experience navigating any of the child and family –serving systems and teaching family members who are involved with the child and family serving systems.
  • Ability to appropriately use personal experience in navigating medical/mental health system
  • Must have exceptional writing and communication skills
  • Bachelor’s degree in human services field from an accredited university and one (1) year of experience working with the target population, or
  • Associate’s degree in a Human Service field from an accredited school and one (1) year of experience working with children/adolescents/transition age youth, and/or high school diploma or GED and a minimum of two (2) years’ experience working with children/adolescents/transition age youth.
  • Must possess a current valid driver’s license and reliable transportation, must have proof of automobile insurance.
  • Provide culturally competent services to ensure that the there is a recognition of economic status, gender, sexual orientation, ethnicity, race, language and the unique values and goals for each youth and family.
  • Willingness and ability to seek consultation and additional services when necessary to overcome barriers impacting the delivery of care

Responsibilities

  • Engaging the parent/caregiver with learning how to network and link to community resources and services for support, this includes how to link resources and treatment providers, advocating for and accessing resources to address the youth’s emotional and Behavioral needs.
  • Work closely with the family and any existing/referring behavioral health providers to ensure implementation of the objectives and goals identified in the treatment plan
  • Participate in all care planning meetings and processes for the youth.
  • Interact with agencies such as (DMH, DCF, DYS, DPH, etc...) to support services/care planning and coordination on behalf of and with the youth and parent/caregiver.
  • Coordinate with and attend all Care Planning Team meetings, and treatment team meetings.
  • Attend weekly CSA staff meetings
  • Report weekly to Senior Parent Partner and keep supervisor updated on all cases
  • Complete all necessary paperwork and data entry assignments in a timely manner
