About The Position

SJCH is seeking a Family Nurse Practitioner (NP) to provide comprehensive, whole person care to patients. In this role, you’ll advance our mission of health equity by delivering high quality, community based primary care in a collaborative, interdisciplinary environment. You’ll work alongside medical, dental, and behavioral health professionals to deliver integrated services that make a real difference in patients’ lives every day.

Requirements

  • Master’s or Doctoral degree from an accredited Nurse Practitioner program
  • Completion of an accredited Family Nurse Practitioner program
  • National board certification (AANP or ANCC)
  • Active, unrestricted California NP license
  • NP Furnishing Number (or eligibility)
  • Graduation from an accredited Physician Assistant program
  • National board certification (NCCPA)
  • Active, unrestricted California PA license
  • Active DEA registration and BLS certification
  • Familiarity with Electronic Health Records (EHR) systems
  • Strong commitment to health equity, cultural humility, and caring for diverse communities

Responsibilities

  • Provide comprehensive primary care for patients across the lifespan
  • Conduct physical exams; order, interpret, and manage diagnostic testing
  • Diagnose and manage acute and chronic medical conditions
  • Collaborate with multidisciplinary teams to deliver integrated, whole person care
  • Educate patients and families on disease prevention, treatment plans, and healthy lifestyle choices
  • Document patient care accurately and timely in the EHR system
  • Support SJCH’s mission through clinical service, outreach, and community engagement
