About The Position
Community Healthcare Network, is a Federally Qualified Health Center (FQHC) serving the underserved communities of Brooklyn, Bronx, Manhattan and Queens. Through the care we provide at our Health Centers and through our various programs, we touch the lives of 85,000 people annually. We are seeking a Family Nurse Practitioner for our Long Island City Health Center located at 36-11 21st Street, Long Island City, NY 11106. Reporting to the Medical Director, this individual will be responsible for providing evidence-based Primary Care to our patients. This individual is an integral of the collaborative care team which includes nursing staff and other services such as Health Education, Nutrition, Social Work and Behavioral Health. Requirements
- valid Family Nurse Practitioner N.Y.S. License to deliver medical care
- at least two (2) year of post-graduate/residency experience delivering care in a Primary Care/Ambulatory Care setting
