About The Position

We are seeking a bold, compassionate Family Nurse Practitioner (FNP) to join a fast -paced, patient -first medical team in Metro Detroit. This role is ideal for a provider who is passionate about delivering high -quality, relationship -based care in a Direct Primary Care (DPC) environment while serving a diverse community.

Requirements

• Active Michigan Family Nurse Practitioner (FNP) license
• Master’s Degree in Nursing or related field
• Bilingual in English and Spanish
• Strong clinical judgment, patient advocacy mindset, and commitment to relationship -based care

Nice To Haves

• 2–5 years of experience in Primary Care
• Experience in outpatient, community -based, or value -based care models
• Comfort working in a fast -paced but patient -focused clinical environment

Responsibilities

• Provide comprehensive primary care services to patients across the lifespan
• Deliver compassionate, culturally responsive care in a patient -centered setting
• Conduct assessments, diagnose conditions, develop treatment plans, and prescribe medications as appropriate
• Educate patients and families on preventive care, wellness, and chronic disease management
• Document care accurately and efficiently using established clinical workflows
• Collaborate with clinical and administrative team members to support continuity of care
• Support communication with patients in both English and Spanish
