Requirements

  • Arizona Nurse Practitioner License
  • Master’s degree in nursing or DNP
  • Possess a valid First Aid/CPR and AED certification
  • Possess a valid DEA license
  • Must have valid Arizona driver’s license, be 21 years of age with minimum 3 years driving experience, and meet requirements of Terros Health’s driving policy
  • Must have a valid Arizona Fingerprint Clearance card or apply for an Arizona fingerprint clearance card (Level 1) within 7 working days of assuming role.
  • Must pass a TB Test and Drug Screen
  • Proficient using computer-based systems including electronic health records (NextGen preferred), Microsoft Outlook, and other Microsoft Office products.

  • NextGen Experience is a PLUS

Responsibilities

  • Conducts physicals/check-ups, evaluations, diagnostics, treats illnesses, orders lab tests, general and preventative care, medication prescribing and monitoring, and individual treatment and case consultation.
  • Educates patients about preventative care, wellness activities and prescribed treatments.
  • Provides leadership and cohesion within the clinic environment, and may provide direct supervision to other professional and/or non-professional medical staff, and students.
  • Participates in program evaluation procedures and professional record keeping, makes referrals appropriately for specialty and behavioral health care.
  • Provides direct patient services not less than 85% of the work time.
  • Employs therapeutic interventions appropriate for identified treatment issues.
  • Seeks appropriate clinical supervision/support, when necessary, to ensure the provision of effective treatment.
  • Establishes appropriate patient files that conform to agency guidelines, best professional practice and State licensing regulations.
