About The Position
Community Health Center (CHC) is hiring a Family Nurse Practitioner for Primary Care in Keyport. CHC offers comprehensive, community-based, preventive, primary care and wellness services for New Jersey residents of all ages at four convenient locations. The health care services are designed to meet the specific needs of the entire family, from annual check-ups and immunizations to sick visits, mental health counseling, and critical prenatal care. In this full-time role, you will be responsible for providing holistic primary care services in collaboration with our Community Health Center team in Keyport. You will help improve patient health outcomes and support the overall well-being of the community.
