About The Position

The Adult Nurse Practitioner provides comprehensive primary care services to adult patients with acute and chronic medical conditions. Responsibilities include conducting physical examinations, developing assessment and treatment plans, ordering and interpreting diagnostic tests, prescribing medications, and counseling patients. Care is delivered in collaboration with a primary care physician and in alignment with NOELA Community Health Center's mission to provide high-quality, patient-centered healthcare in a culturally diverse setting.

Requirements

  • Master's degree in nursing with a specialization in Adult Care (required).
  • Possess prescriptive authority.
  • Certified by the State Board of Medical Examiners.
  • Board certified by NCCPA, NBDPA, or other recognized certifying board.
  • Proficiency in physical assessment.
  • Ability to use standard computer applications, including word processing software.
  • Excellent oral and written communication skills.
  • Ability to work effectively in a culturally diverse, patient-centered, holistic environment.

Nice To Haves

  • At least one year of full-time clinical nurse practitioner experience preferred.
  • Language skills in Spanish or Vietnamese help serve our community, but are not required for this position.
  • Flexibility to work in multiple clinical settings strongly preferred.

Responsibilities

  • Provide healthcare services to patients presenting to NOELA CHC or affiliated sites.
  • Conduct comprehensive medical evaluations and develop individualized plans of care.
  • Initiate and manage therapeutic interventions independently or in consultation with a physician as indicated.
  • Order, perform, and interpret screening and diagnostic tests; recognize abnormal findings and develop appropriate follow-up plans.
  • Provide patient education and contribute to group health education efforts.
  • Conduct nurse case management and patient triage (in-clinic and via telephone).
  • Communicate relevant clinical information clearly with team members to ensure quality care.
  • Report unusual conditions or patient reactions to the supervising physician.
  • Complete documentation of all patient encounters in a timely and accurate manner.
  • Participate in the implementation of clinic-wide continuous quality improvement programs.
  • Identify and refer patients to appropriate community resources; provide guidance on utilization.
  • Contribute to the development of improved community resources and care delivery patterns.
  • Cross-train with support staff to assist as needed in patient care.
  • Demonstrate a willingness to learn new tasks and seek clarification before beginning unfamiliar assignments.
  • Participate in continuing medical education (CME) programs to maintain clinical knowledge and licensure requirements.
