Requirements

• Licensed Registered Nurse in Michigan
• Master’s degree from an accredited Nurse Practitioner program
• Certified Family Nurse Practitioner (FNP)
• Active NP specialty certification in Michigan
• Graduate of an accredited Physician Assistant program
• NCCPA certified
• Active Michigan PA license
• Active DEA license (or ability to obtain)
• Commitment to serving diverse and underserved populations

Nice To Haves

• One or more years of family practice experience preferred
• Bilingual English/Spanish preferred, but not required

Responsibilities

• Provide primary care services within scope of practice
• Assess, diagnose, and manage acute and chronic health conditions
• Develop patient-centered treatment plans and provide education
• Collaborate with physicians and care team members to ensure continuity of care
• Accurately document and code patient encounters in the electronic health record
• Participate in peer review, quality improvement, and clinical team meetings
• Work collaboratively with interpreters and community partners
• Support NMHSI’s Medical Home Model and organizational initiatives
