About The Position

This is a full-time position working between the Health Center in La Grange Park and BEDS Plus in Summit. The Family Nurse Practitioner or Physician Assistant works under the direct supervision of the Chief Medical/Population Health Officer, who is also the collaborating physician. FNP/PA completes all the duties associated with their clinical license and manages patient care in accordance with the professional guidelines of Pillars Community Health. This position is unique as it is a collaborative position between Pillars Community Health and Beds-Plus. The objective is to optimize health for those adults experiencing housing insecurity by improving access to primary care FNP/PA will spend 50% of their time providing medical assessment and care in a clinical location at the Summit Service Center, which is a dedicated location for individuals experiencing homelessness, and 50% at the PCH Health Center in La Grange Park. Instructs, oversees, and works alongside the RN's and Medical Assistants in the delivery of care. Supports the policies, procedures and protocols and plans of the Health Center as it relates to direct patient care, care management, development and implementation of clinic procedures and patient education. A quality assurance element is achieved in collaboration with the Quality Improvement Specialist. Embraces and acts upon the clinical goals and approach of the Chief Medical/Population Health Officer. As a quality measure, assures accurate E.H.R management, and HIPAA and OSHA compliance.

Requirements

  • Must hold an active and unrestricted Advanced-Practice Nurse or Physician Assistant license in the state of Illinois, after having completed a graduate-level or higher nursing or physician assistant accredited program and a professional certification from an accredited educational institution.
  • Current CPR and DEA certifications and Medicaid and Medicare provider numbers required for credentialing and privileging
  • At least 2 years of experience in ambulatory medicine care and/or community health experience. FQHC preferable
  • Analytical & Critical thinker
  • Strong time management skills and computer proficiency in E.H.R preferred
  • Highly organized
  • Ability to multi-task and willingness to learn new tasks
  • Ability to work within a team environment
  • Empathic and sensitive to patient needs
  • Demonstrate strong case management skills for patient care
  • Excellent communication skills
  • Must maintain a safe driving record and have current valid Illinois Driver's License.
  • Personal vehicle is needed for transportation to/from BEDS Plus locations and Pillars Community Health sites.

Nice To Haves

  • Interest/experience in Medication Assisted Recovery (MAR) preferred.
  • Experience working with Medicare population also helpful.
  • Experience in patient education, in areas of diabetes management, cardiac risk assessment and/or family planning is helpful.
  • Bilingual (English/Spanish) preferred.

Responsibilities

  • Provides general medical care and treatment to patients under the direction of a physician and as dictated by their Illinois Clinical License and PCH policies and procedures
  • Performs physical examinations and preventive health measures within prescribed guidelines and instructions of physicians to diagnose and treat acute, chronic, and long-term health issues of patients.
  • Orders, interprets, and evaluates diagnostic tests to identify and assess patient's clinical problems and health care needs.
  • Prescribes or recommends medication or other forms of medical treatment such as therapeutic procedures.
  • May refer patients to physicians for consultation or to specialized health resources for treatment.
  • Coordinates patient treatment, management and planning with the clinical staff and physician to provide continuity of care, including assistance with referrals to other agencies and health care providers.
  • Submits health care plan and goals of individual patients for periodic review and evaluation by physician.
  • Responsible for documentation of patient care; records physical findings, and formulates plan and prognosis, based on patient's condition.
  • Provides education to patients and/or their families about preventive care, medical issues and the use of prescribed medication and treatments.
  • Assists in the compilation of statistical data and patient outcomes for agency needs, as requested.
  • Attend all meetings as required
  • Completes encounter requirements as established by HRSA (Health Resource & Service Administration) for FQHC's (Federally Qualified Health Centers)
  • Adheres to OSHA guidelines for safe work environment
  • Works with students and interns to provide evaluation and care to patients
  • Participates in continuing professional development to keep informed of clinical practices and assimilates this knowledge into clinical practice
  • Maintains compliance with health center immunization requirements
  • Performs other duties as assigned
