Requirements
- Education: Master’s or Doctoral degree from an accredited FNP program
- Certification: National certification as a Family Nurse Practitioner
- Licensure: Eligible for Connecticut APRN licensure
- Experience: Prior primary care experience preferred; new graduates welcome
Responsibilities
- Provide comprehensive primary care services to patients across the lifespan
- Diagnose and manage acute and chronic medical conditions
- Conduct preventive care visits, annual physicals, and wellness screenings
- Prescribe medications and develop evidence-based treatment plans
- Collaborate with physicians and multidisciplinary care teams
- Maintain accurate and timely EMR documentation
- Educate patients on health maintenance and disease prevention
