The Family Nurse Practitioner is responsible as a provider of primary health care services who coordinates and refers clients for health care and health education. Requirements

  • Nurse Practitioner, unencumbered licensure with State of Oregon as Certified Nurse Practitioner and prescriptive authority.
  • Basic Life Support within introductory period.
  • Evidence of experience sufficient for successful performance of duties noted above.
  • Ability to maintain professional boundaries with clients, the community, and other staff members.
  • Ability to pass a pre-employment or for cause drug tests.
  • Ability to pass criminal background and DMV checks.

Nice To Haves

  • Experience working within the Native American/Alaska Native community is considered a plus.

Responsibilities

  • Provide comprehensive primary health care.
  • Coordinate patient care with other providers and agencies and advocate for client within health care systems.
  • Provide services in prenatal integrated health care of high-risk chemically dependent pregnant patients, in consultation and/or collaboration with obstetrical health care provider.
  • Perform initial and ongoing health risk assessments and counseling for high-risk lifestyle behaviors and/or familial diseases: ongoing drug and alcohol assessment and HIV screening.
  • Encourage family participation in health care of patient as deemed appropriate by patient and other clinic providers.
  • Develop and document plan of care and outcomes evaluations.
  • Coordinate with contractual pharmacy, primary care providers, hospitals, and laboratories for required diagnostic procedures and delivery/births.
  • Provide health education to individual patients as needed, such as birth control, safer sex, and high risk behavior and health consequences.
