Requirements

  • Valid Nurse Practitioner license in Texas.
  • Experience in primary care or family medicine.

  • Knowledge of electronic health record systems.

Responsibilities

  • Conduct medical history taking, exam, diagnosis, and treatment.
  • Interpret lab work.
  • Perform routine outpatient procedures for which they have adequate training and experience.
  • Counsel patients and provide patient education.
  • Assist in chart review and policy and procedure development.
  • Conduct and document PDSAs for quality improvement.
  • Integrate and employ Care Model and Performance Improvement Model in delivery of care.
  • Provide comprehensive primary healthcare services to patients of all ages.
  • Conduct thorough patient assessments, including medical history, physical examinations, and diagnostic tests.
  • Diagnose and treat acute and chronic illnesses, injuries, and common medical conditions.
  • Develop and implement personalized treatment plans based on evidence-based guidelines and patient needs.
  • Manage and coordinate patient care, including prescribing medications and ordering diagnostic tests.
  • Collaborate with interdisciplinary healthcare team members to ensure holistic patient care.
  • Coordinate referrals to specialists and follow up on patients' progress.
  • Perform routine health maintenance and preventive care, including health screenings and vaccinations.
  • Promote health promotion initiatives, including counseling on nutrition and exercise.
  • Document patient encounters and maintain accurate medical records.
  • Stay updated on current healthcare practices and trends.
  • Uphold professional and ethical standards.
  • Contribute to quality improvement and research initiatives.
