About The Position

The Family Nurse Practitioner (APP-FNP) at Southeast Health Center is responsible for delivering high-quality care for the diagnosis and treatment of disease, promoting health, and preventing disease. This role involves providing physical and emotional support to patients and families, working with both new and established patients in accordance with licensure and the highest standards of quality and integrity. The position requires a high level of independent judgment and decision-making, with the incumbent expected to perform all essential functions and responsibilities assigned within the organization. The Advanced Practice Provider must maintain compliance with various regulatory standards and organizational policies while delivering compassionate, patient-centered care.

Requirements

  • Master's or Doctorate's degree from an accredited Advanced Practice Nursing Program.
  • Active and unencumbered Advanced Practice Registered Nurse (APRN) Licensure in Arizona.
  • Active Board Certification as a Nurse Practitioner in specialty area.
  • Active DEA license or ability to apply for it prior to or upon hire.
  • Current provider card in Basic Life Support (BLS) and advanced life support certifications if relevant.

Nice To Haves

  • Doctorate's degree from an accredited Advanced Practice Nursing Program.
  • One or more years of experience as an Advanced Practice Provider in an acute, outpatient, or ancillary healthcare environment.
  • Clinical experience working in a Community Health Center Environment.

Responsibilities

  • Delivers healthcare in a professional manner that meets or exceeds criteria set by regulatory agencies.
  • Evaluates patients, performing examinations, and establishing preventive health practices.
  • Analyzes reports, test results, medical records, and examinations to diagnose conditions and propose treatment options.
  • Provides advice to patients for lifestyle and diet changes to improve health.
  • Facilitates consistent, coordinated care and clear communication among healthcare team members.
  • Serves as an advocate for patients and families in navigating the healthcare system.
  • Prescribes medications and treatment regimens for identified medical conditions.
  • Maintains medical records in compliance with regulatory requirements.
  • Attends to patient emergencies in clinic and hospital environments.
  • Complies with payor contracts and legal requirements regarding care standards.
  • Maintains good standing with Federal and State agencies.
  • Participates in ongoing Continuing Medical Education Programs.
  • Actively seeks opportunities for clinical advancement and skill attainment.
  • Represents El Rio Health as a professional role model in the community.
