About The Position

We are hiring for: Family Model Provider Type: Family Model Provider (TN) - Independent Contractor. If so, you may find fulfillment in the role of a Family Model Provider. Summary: Family Model Provider: We are looking for caring individuals who share our dedication to providing a supportive environment for persons with intellectual and /or Developmental Disabilities (I/DD) in their home. We ask that you bring a desire to serve others and share your home, and RHA will provide the training and support to ensure your success. As a Family Model Provider, you will be asked to provide support for individuals with disabilities in a home setting. You will be asked to provide the supports identified below based on the needs of the individual in your care.

Requirements

• HS Diploma/GED preferred
• 18 years old or older
• Documentation of home ownership: current mortgage statement or renter’s lease
• Proof of homeowner’s or renter’s insurance or personal property insurance
• Valid Driver’s License; Reliable transportation with valid vehicle insurance
• Ability to pass criminal background and Registry checks as well as a Drug Screen
• TB Screenings for all members of household
• Willing to be a self-employed worker

Responsibilities

• Seek to understand the individual in the context of their personal history, their social and family networks, and their hopes and dreams for the future.
• Support the individual in eating nutritious meals and snacks consistent with dietary needs, resting (by providing sleeping accommodations), daily self-care activities, leisure/recreation, education and working.
• Prepare, teach and assist the person in food preparation, and cleaning of the home.
• Honor the choices, preferences and culture of the person you support.
• Assist in providing opportunities for social, emotional and spiritual needs to be met.
• Assist with transportation and scheduling of all medical, therapy, vocational and recreational activities.
• Ensure that the home environment is safe and in good condition.
• Implement services and supports to address goals identified through the Person Centered Plan.
• Assist the person supported by attending team meetings and facilitating communication related to individual needs, response to service and supports, and progress toward goals.
• Follow state guidelines specific to Family Model Services.
