We are seeking a practitioner who shares this commitment and is looking for a long-term placement, ensuring our patients continue to receive care from familiar faces. We are seeking an enthusiastic and compassionate Family Medicine Practitioner, either a Nurse Practitioner (NP) or Physician Assistant (PA), to join our team. The ideal candidate will be passionate about rural healthcare and committed to going above and beyond to meet the needs of our patients. This role requires a practitioner who is efficient, resourceful, and dedicated to providing the highest level of patient care.

Requirements

  • Current licensure as a Nurse Practitioner (NP) or Physician Assistant (PA) in the state of Indiana.
  • Strong communication and interpersonal skills.
  • Ability to work independently and as part of a collaborative team.
  • Passion for patient education and a commitment to improving patient outcomes.
  • Interest in a long-term placement to provide continuity of care for our patients.

Nice To Haves

  • Previous experience in a family medicine or rural health setting is preferred but not required.

Responsibilities

  • Provide comprehensive primary care services to patients in a rural setting.
  • Educate patients and their families about health conditions, treatment plans, and preventive care.
  • Assist patients in navigating healthcare resources, ensuring they receive the care they need, either in-house or through referrals.
  • Provide in-house lab testing and perform a variety of procedures to meet patient needs.
  • Collaborate with our healthcare team to deliver holistic, patient-centered care.
  • Maintain accurate and detailed patient records, ensuring compliance with all regulations and standards.
  • Contribute positively to our family-oriented workplace culture, working collaboratively with all team members.
