Position Summary The Family Medicine Physician provides comprehensive, patient-centered primary care services to individuals and families across the lifespan within a community health center setting. This role focuses on delivering high-quality, culturally competent care to a diverse and often underserved population, regardless of ability to pay. The physician is responsible for diagnosing and treating acute and chronic conditions, providing preventive care, managing overall patient health, and coordinating care with interdisciplinary teams. The position emphasizes a holistic approach that integrates medical, behavioral, and social determinants of health to improve patient outcomes. As part of a mission-driven organization, the Family Medicine Physician supports the health center’s commitment to accessible, equitable healthcare and participates in quality improvement initiatives, care coordination, and community outreach efforts. The physician may also contribute to clinical leadership, mentoring, and the advancement of patient-centered medical home (PCMH) standards.

Requirements

  • Doctor of Medicine (MD) or Doctor of Osteopathic Medicine (DO) from an accredited institution
  • Board Certified or Board Eligible in Family Medicine
  • Current, unrestricted medical license in the state of practice
  • DEA certification
  • Knowledge of advanced principles of health promotion, prevention, and motivation
  • Knowledge of medical terminology and appropriate abbreviations
  • Demonstrates knowledge of the Texas Medical Practice Act
  • Ability to document, with clarity, all information relevant to a patient’s needs
  • Ability to use electronic health records
  • Excellent interpersonal and written communication skills

Nice To Haves

  • Experience in a community health center, FQHC, or underserved setting preferred
  • Experience with diverse patient populations across the lifespan
  • Familiarity with integrated care and team-based models preferred
  • Clinical fluency in Spanish
  • Knowledge of public health principles and practice

Responsibilities

  • Management of Patient Health Deliver a high standard of patient care in general practice, using advanced autonomous clinical skills, and a broad and in-depth theoretical knowledge base.
  • Provide a first point of contact within the Practice for patients presenting with undifferentiated, undiagnosed problems, making use of skills in history taking, physical examination, problem-solving and clinical decision-making, to establish a diagnosis and management plan.
  • Initiate necessary diagnostic tests or investigations and interpret findings/reports.
  • Prescribe safe, effective and appropriate medication as defined by evidence-based guidelines.
  • Prioritize health problems and intervene appropriately, including initiation of effective urgent or emergency care with appropriate referral as needed.
  • Promote health and wellness using health promotion, health education, screening and therapeutic communication skills.
  • Clearly communicate the health status of the patient using appropriate terminology, technology and format.
  • Provide safe, evidence-based, cost-effective, individualized patient care.
  • Work within the practice guidelines, policies and protocols.
  • Maintain accurate and complete records that are prepared in a timely manner, utilizing computer systems where appropriate.
  • Provide evening and weekend call, if appropriate, on a rotational basis with other physicians according to a schedule approved by the CMO.

  • Collaborate with the clinical support team and others in developing new roles, responding to changing healthcare needs.
  • Facilitate and participate in multi-disciplinary education and clinical supervision.
  • Participate in organizational decision making, interpret variations in outcomes and use data from information systems to improve practice.
  • Participate in continuing professional development opportunities to ensure that up-to-date evidence-based knowledge and competence in all aspects of the role is maintained.
  • Monitor the effectiveness of clinical practice through quality assurance strategies such as the use of chart audits and peer review.
  • Play an active role in the development of practice development plans and new services.
  • Have knowledge of and ensure compliance with all policies, procedures, and federal, state, and local rules and regulations as applied to the practice of medicine.
  • Team Role As a clinician, be available to support other healthcare team members and to accept referrals from other team members.
  • Work cooperatively with all members of the Primary Health Care Team.
  • Value all team members.
  • Delegate appropriate tasks to appropriate team members.
  • Support the delivery of health and safety guidance as agreed within the practice.
  • Support the writing of, maintenance and delivery of quality guidelines within the practice.
  • Organizational Complete records, audits, reports and respond to appropriate questions and requests.
  • Attend meetings as requested, particularly monthly medical staff meetings.
  • Support effective communication channels between the whole team.
  • Develop a sound understanding of eClinicalWorks (eCW); event scheduling, session planning and essential practice coverage needs.
  • Keep the CMO informed about difficulties or problem areas when aware.
  • Contribute to practice strategy and developments when requested.
  • Work according to clinical quality recommendations and support the Quality Committee initiatives.
  • Understand the role of the Risk Management Team within the practice, contribute to risk assessment, incident reporting and uphold the tenants of infection control.
  • Actively participate in the delivery of productivity and revenue targets.
  • Risk Management The physician must comply with JFS Dallas’ Risk Management Plan and related policies, in particular by following agreed upon safe working procedures and report incidents using the organizations incident reporting process.
  • Specifically, they must: Comply with any safety instructions; Use in a proper and safe manner, the equipment and facilities provided; Refrain from willful misuse or interference with anything provided in the interest of health and safety; Report as soon as practical any hazards and defects; Report as soon as practical, accidents and untoward occurrences and ensure that accident forms are completed.
