Family Medicine Physician - Apple Valley
About The Position
Fairview is seeking a Family Medicine physician to join our primary care practice at M Health Fairview Clinics - Apple Valley. Join our busy practice which has been in the community for over 20 years. Practice Details: 11 provider primary care practice with more than 5 specialists onsite. Patient encounters average 18-22 patients per day. 34 patient contact hours per week. Call Frequency: 1:10 low intensity - phone consultation only with first tier nurse triage Openings: 1.0 FTE (Full Time) - Flexible schedule. Examines patients, formulates differential diagnostic plans, defines and orders required diagnostic testing. Provides preventative, chronic disease, and acute care depending on the need of the clinic site. Ensures proper injury care and disease prevention, diagnosis, treatment, and recovery. Prescribes and administers medications, performs routine vaccinations, and provides advice regarding personal health and hygiene. Interprets examination findings and test results and implements treatment plans. Refers patient to medical specialist or another practitioner when necessary. Completes medical record and electronic order management (via in basket); and completes cross coverage depending on site needs. Follows organization policies and procedures and protocols. Actively participates in multidisciplinary team approach to case management Participates in utilization and quality tools and activities that promote continuous improvement in delivery of patient care, patient outcomes or workflows. Maintains knowledge of, and complies with, all relevant laws, regulations, policies, procedures, and standards. Actively participates in creating and implementing improvements
Requirements
- Graduate of an accredited ACGME/AOA Family Medicine residency training program
- BC/BE with the ABMS or AOA
- Licensed or ability to obtain licensure through the Minnesota Board of Medical Practice
- Ability to obtain and/or maintain DEA certification for the State of Minnesota
- Meet threshold criteria/qualification for Credentialing and Privileges
- Board Certified in Family Medicine, or dual Board Certification in Internal Medicine/Pediatrics
- Minnesota license to practice as an MD or DO
