Position Summary ChesPenn is seeking a dedicated and patient-centered Family Medicine Physician to provide comprehensive primary care to adults and children at our Chester, PA location. This role is ideal for a physician who thrives in a collaborative, community-focused environment and is passionate about serving medically underserved populations.

Requirements

  • Graduate of an accredited medical school
  • Completion of an approved residency program in Family Medicine
  • Unrestricted Pennsylvania medical license
  • Board eligibility required; board certification preferred
  • DEA certification
  • Current COVID-19 and flu vaccinations required
  • Strong written and verbal communication skills
  • Excellent diagnostic and critical thinking abilities
  • Positive, inclusive, and patient-focused demeanor
  • Ability to work effectively as part of a multidisciplinary healthcare team
  • Ability to meet approved physical and medical standards

Nice To Haves

  • Experience in ambulatory primary care or community health preferred
  • Familiarity with trauma-informed care principles preferred
  • Valid driver’s license and access to a licensed automobile preferred

Responsibilities

  • Provide routine and preventive care, including physical exams and screenings
  • Manage acute and chronic conditions in an outpatient setting
  • Conduct follow-up visits for patients referred to specialists
  • Educate patients on health maintenance and ongoing care
  • Collaborate with nurses, medical assistants, and administrative staff
  • Supervise nurse practitioners and physician assistants
  • Participate in emergency call coverage rotation, including weekends and holidays
  • Maintain patient confidentiality and professional conduct at all times
  • Perform additional duties as assigned by supervisor
  • Maintain productivity levels in accordance with BPHC grant standards
  • Meet targets for patient encounters and visits per patient annually
  • Document patient care accurately in the Electronic Medical Record (EMR) system
  • Build relationships with community agencies and hospitals
  • Lead or support medical and public health initiatives as needed
  • Contribute to the development of the annual healthcare plan
  • Engage in continuing medical education and professional development
